The NYS Education Department Commissioner's Regulations for School Health Services [Section 136 CR 136.3] requires new students and students entering pre-k, kindergarten and grades 1,3,5 and 7 to have a current health certificate on file. If you have any questions, please contact Mrs. Hollander at 693-5604 ext.112.
The school districts listed below have provided the transportation form to request transportation for the 2024-25 school year. Please click on your home school district's form to view and print. If it is not listed, you must go on your district's website or call them directly for information.
Note: Forms are to be sent directly to the public school district - NOT returned to the St. Christopher School office.
